I understand that The Montessori Child Development Centre ( Day Care and OSC) reserves the right to refuse or discontinue enrolment at their discretion should they feel my child poses a health or safety concerns, displays unmanageable behaviour issues and is not suited for the program. I also certify that I have received, read, fully understood and agreed to all the information as stated in The Montessori Child Development Centre Parent Handbook. I also understand that it is my responsibility to keep my subsidy on child’s fee up–to-date otherwise in case of cancelation of the subsidy for any reason I will be responsible for paying the fee in full. I hereby certify that the information on this form is correct and I will inform the director to update it if there will be any change in any of the information in future. Furthermore, I understand that I am required to give 30- day written notice to the Director in case of withdrawal from the program..
